“We are deluged by misleading online hype, oftentimes not justified by the factual data. This goes in the very opposite direction we should take to plug the gap of knowledge.”
Key takeaways
For more than three decades, the professor has worked where laboratory discovery meets patient care—studying mesothelioma’s immune biology, molecular vulnerabilities, resistance mechanisms and the strength of the evidence behind its treatments.
Research philosophy
Editorial framingRigor is not separate from access. Independent evidence, adequate funding and patient selection shape which discoveries can become meaningful treatment.
“Unfortunately low incidence of mesothelioma means less financial incentive for drug companies, and lower funding for the research.”
The p53-model precision-medicine study, reported in the Journal of Translational Medicine (October 2018) and the International Journal of Molecular Sciences (January 2019):
“This study offers a good approach to test the real impact of genetic studies in precision medicine… we believe it will have a potential for use in Mesothelioma treatment by identifying patient sub-populations that are likely to respond to particular drugs.”
The through-line
From the immunologic behavior of mesothelioma cells to the fragility of trial evidence: a research record defined by questions that move between bench and bedside
Early work established that mesothelioma cells can present antigens—part of the immunologic basis for understanding the disease.
Research connected BAP1 status with gemcitabine sensitivity, advancing a biomarker-driven view of treatment response.
A critical review of immunotherapy trials separated clinical promise from demonstrated patient benefit.
A comparative-effectiveness study re-analyzed three pivotal trials: MPS (2003), MAPS (2016) and CheckMate 743 (2021), examining the statistical fragility of their findings in a JAMA Network Open SHRO/GIMe analysis.
“For all three studies analyzed, there is not really evidence to conclude that any of them improve [malignant pleural mesothelioma (MPM)] patients’ survival.”
“how much can we trust the results of large sponsored clinical trials, when the benefit obtained is minimal…”

As corresponding author, with affiliations at UniCamillus and GIMe, Mutti reports a re-analysis of the CONFIRM and PROMISE-meso trials that found the early CONFIRM overall-survival signal coincided with short follow-up and post-progression censoring patterns, with no OS benefit in the longer-follow-up data.

Medical articles
International Journal of Cancer · PMID 9833768
This preclinical in vitro study examined four primary cultures of human malignant mesothelioma cells. The cells expressed antigen-presentation-associated molecules and, after pulsing with recall antigens, induced proliferation of autologous lymphocytes; PPD-pulsed cells stimulated a PPD-specific CD4+ T-cell clone through an MHCII-DR-mediated process.
Journal of Pathology · PMID 11276005 · preclinical research that provided the rationale for chemotherapy plus antiangiogenic therapy, now front-line treatment in many countries
In this preclinical study of human malignant pleural mesothelioma, cell lines, biopsies and patient effusions were analysed for VEGF signalling. Mesothelioma cells expressed VEGF, Flt-1 and KDR; recombinant VEGF increased proliferation in vitro and neutralising antibodies reduced it, while higher serum VEGF levels were associated with shorter patient survival.
European Respiratory Journal · PMID 19717482
This ERS/ESTS Task Force practice guideline synthesised expert guidance for managing malignant pleural mesothelioma, covering diagnosis, staging, prognosis and therapeutic strategies. It highlighted limitations in diagnostic staining and staging validation, and noted limited evidence on optimal multimodal treatment and the need for prospective study.
The Lancet Oncology · PMID 24035405 · co-last author · the first immune checkpoint inhibitor trial in mesothelioma
This open-label, single-arm phase 2 trial enrolled 29 patients with unresectable advanced malignant mesothelioma progressing after platinum-based first-line treatment, who received tremelimumab every 90 days. Two patients (7%) had durable partial responses and 31% achieved disease control; the prespecified 17% objective-response target was not met.
Immunotherapy · PMID 28231719 · co-last author
This special report reviews clinical trials of tremelimumab in unresectable malignant mesothelioma, addressing clinical efficacy, safety and tolerability. It concludes that biological and clinical considerations did not support tremelimumab monotherapy, and that evidence for immune checkpoint inhibitors in mesothelioma remained limited and uncertain.
European Respiratory Journal · PMID 32451346
This multidisciplinary practice guideline updated the 2009 ERS/ESTS recommendations for malignant pleural mesothelioma through a systematic review of 2009–2018 evidence, appraised with GRADE. It addressed diagnosis, pathology, staging, prognostic assessment and treatment, noting limited efficacy of (chemo)therapy, restricted suitability for radical surgery, and limited data on optimal multimodal treatment.
JAMA Network Open · PMID 35262715
This comparative effectiveness study examined MPS, MAPS and CheckMate743, three phase 3 frontline randomised trials in malignant pleural mesothelioma. Reconstructed survival analyses found no statistically significant difference between the MAPS and CheckMate743 intervention groups and identified low survival-inferred fragility indices and differential censoring in CheckMate743, factors that may affect interpretation of the original results.
